Forms

Add/Drop Form - A student may drop a course within the first two weeks of the term and no entry will be made on the permanent record. An add/drop form is to be used for this purpose and must be picked up at the Registrar's office, rm 304.

A withdrawal after the first two weeks of class must be done by petition, requiring the assent of the instructor, the advisor, and the Dean. Withdrawal after the fourth week of the semester is permitted only under special circumstances.

The rate of refund is as follows:
1. Through the end of the second week of class Full Refund
2. Through the end of the third week of class 70% Refund
3. Through the end of the fourth week of class 50% Refund
4. After the fourth week, no refund and course stays on transcript with entry of “W”.

Courses may be added, with the permission of the instructor, up to the end of the first week of class.

ACTS CROSS REGISTRATION AND GRADE REPORT FORM.
ACTS students use this form to register at another ACTS institution. Download this Word Document and complete the form fields. Then the completed form must be sent to the Registrar for the school for which you are Cross-registering via your official school email account (Example: youremailname@ctu.edu).

ACTS Equivalent Substitution for LSTC Required Course (html)
/ACTS Substitution (word file) /ACTS Substitution PDF / Students substituting an equivalent ACTS course for an LSTC Core Required must complete this form and submit it along with your ACTS Cross-registration form at the time of registration (also write the ACTS course on your registration form).

Adjustment of Academic Program Petition - The student should use this form to request permission to transfer credit, receive special credit, substitute a course, change a grade, etc. Student must obtain the signatures required on the form, including the Dean’s approval. The form must be submitted to the Registrar after all signatures have been obtained.

Bi-register at the Lutheran School of Theology at Chicago (for UC students) - Bi-registration allows students at the University of Chicago-Divinity School to register for courses at the Lutheran School of Theology. Read also the instructions.

Bi-register at the University of Chicago (for LSTC students) - Bi-registration allows students at the Lutheran School of Theology at Chicago to register for courses at the University of Chicago-Divinity School. All LSTC students wishing to take courses at the University of Chicago must complete this form and have it signed by the Dean (M.A., M.Div. students), or by the Director of Graduate Studies (Th.M., Ph.D. students). In addition, a copy of the Bi-Registration form must accompany the student's LSTC registration card. The original Bi-Registration form is to be used to complete the registration at the University of Chicago. Read also the instructions.

Change of Degree Program Petition

Core Course Credit Exam Registration Form

Independent Study Petition for Master of Arts, Master of Divinity, and ThM/PhD/D.Min - To be completed and submitted with registration card at time of registration. Title of course and instructor’s name must be written on registration card. A course number will be entered by the registrar. LSTC Substitution Form. Students substituting an equivalent LSTC course for an LSTC Core Required course must complete this form and submit it at the time of registration. | Download in Word format / PDF .

M.A. Concentration Petition (html) / Word file / PDF (description follows:)

M.A. candidates may develop an area of concentration in any discipline represented by the LSTC faculty. The professor or professors in a specific field determine which courses are essential for a concentration in that field and identify pertinent cognate courses. In order to develop a concentration an M.A. candidate must first complete the M.A. CONCENTRATION PETITION FORM, which must be signed by the student's academic advisor and the Director of the M.A. program. The completed form is filed in the Registrar's Office. A concentration requires six courses which may include courses taken to fulfill M.A. requirements. At least four of the courses must be electives 400 level or above. The electives may include cognate courses from other theological disciplines. However, in order to be counted toward the concentration such cognate courses require the completion of the M.A. CONCENTRATION EQUIVALENCY FORM, which is signed by the student's academic advisor and the Director of the M.A. program and filed in the Registrar's Office. The Registrar monitors the student's progress in fulfilling the concentration requirements and will record the concentration on the student transcript when all requirements have been successfully completed.

PASS/FAIL OPTION - This form gives one the option to take course(s) on the basis of Pass/Fail. The form Must be submitted to the Office of the Registrar not later than the Friday of the end of the second week of class--no exceptions. Download the Word form / PDF form.

Thesis Proposal for Master of Arts and Master of Divinity - To be completed and submitted with registration card at time of registration.
